Insurance Information

We here at Ocala Health Surgical Group will be glad to assist you with any questions or concerns that you may have regarding the filing of your insurance claims. Ocala Health Surgical Groupwill file your primary insurance, as well as your secondary insurance, if applicable. We currently participate with Medicare, in addition to most major commercial insurance plans.

Please feel free to contact our office (352) 291-2400 or your insurance provider to confirm that we are a participating provider for your specific plan. At your initial appointment with us, please bring your insurance card(s) with you so that we may retain a copy on file. Additionally, please notify the office promptly regarding any insurance changes you may have. Please note that some commercial plans require pre-authorization before services will be covered.

Payment Information

We require payment of any co-payments, deductibles, and past-due account balances at the time of service. If you do not have insurance, full payment is required at the time of service.
